An application for approval under paragraph MIR-3.4.1 shall be made by using Form 3: Application for Approved Person Status, included under Part B of Volume 6 and the CBB may require the member to furnish it with such information or documents as the CBB considers necessary in relation to the application referred to in section MIR-3.1 and the member shall furnish such information or documents as required by the CBB, licensed exchange and/or licensed clearing house.
